New Patient Information

We welcome new patients that are referred by their veterinarian or self-referred. Providing us with all pertinent medical records for your pet helps us have all the necessary information for our initial evaluation. Having a clear and thorough picture of your pet’s medical history and home environment will assist us in providing you with a rehabilitation plan best tailored to your pet. When you make an initial appointment at Oak Tree Animal Rehabilitation, we will request that you send us the medical records in advance to allow us time to review them before the appointment. You can also provide us with your veterinarian’s contact information and we can obtain the records directly. Please be sure to include medical exam notes, blood tests, radiographs, radiology reports, surgery notes, medications being used and any other information that may be useful for us to understand your pet’s medical history. Please print the forms needed for your appointment below and bring them with you to your pet’s initial evaluation. If you are unable to print them, we will provide you with a copy to fill out at the appointment. Please plan to arrive 15 minutes before your scheduled appointment time to fill out the necessary forms.
